-Accept or refuse any treatment based on the possible risks, benefits, and alternatives, unless you are deemed incompetent (unable to make decisions), and as long as your decision will not harm others (for example, some contagious diseases), as set out by law.
-Express your concerns and get answers to your questions without affecting your care.
-Request the transfer of your health records to another medical professional; you may be charged a fee.
-Be advised of the fees not covered by OHIP that you are responsible to pay.
-Patients are responsible for treating front staff and providers with respect.
-Patients are responsible for providing correct and complete information about their health and past medical history.
-Patients are responsible for keeping their contact information up to date with the front office. They are also responsible for providing at least one (preferably two) emergency contacts so that if the patient cannot be reached in a timely manner to review abnormal results, then the emergency contacts will be contacted to help reach the patient.
-Patients are responsible for requesting results of any labs/imaging tests ordered by contacting the office for an appointment to discuss results if they have not received them.
-Patients are responsible for reporting changes in their general health condition, symptoms, or allergies to their doctor.
-Patients are responsible for reporting if they do not understand the planned treatment or their part in the plan.
-Patients are responsible for following the recommended treatment plan to which they have agreed.
-Patients are responsible for keeping appointments.
-Patients are responsible for following facility rules regarding not wearing perfumes/cologne, not smoking, wearing masks, completing covid screening prior to in person visits, and avoiding inappropriate use of electronic equipment.
-Patients are responsible for what happens if they refuse the planned treatment.
-Patients are responsible for paying for third party services not covered by OHIP.
